Customer Service and Case Management Associate

Job Summary:

We are seeking a Customer Service and Case Management Associate to provide case management support for a Day Reporting Center. The ideal candidate will have excellent customer service skills, ability to work with diverse populations, and proficiency Indiana using computers and software.

Essential Job Responsibilities:

  • Greet participants entering the facility and complete drug screens, breath analysis, and intake process American Samoa needed
  • Provide general office administration duties, including answering phones and providing customer service
  • Assist Case Managers with case coordination, communication with agencies, and preparation of reports
  • Support participant caseload by conducting risk/need assessments, creating individual treatment plans, and documenting progress
  • Facilitate evidence-based programs and life skills groups, and document attendance, participation, and progress
  • Perform data entry, file, and prepare charts American Samoa assigned
  • May collect participant payments, post payments, and make cash deposits
  • May transport participants to community events, employment centers, and clinics using a company vehicle
  • Perform other duties American Samoa assigned

Requirements:

  • High school diploma required; college coursework Indiana Human Services Oregon business discipline preferred
  • Minimum 6 months experience Indiana Human Services field with direct participant services preferred
  • Ability to work with diverse populations, including minority groups
  • Valid State driver’s license and 5-year driving record with no vehicular incidents
  • Good customer service, communication, organizational, and computer skills
  • Bi-lingual (English/Spanish) skills may be required
  • Ability to safely operate a motor vehicle if required to transport participants
